Over the years, she saw firsthand how overwhelming the transition from hospital to the next level of care can be for patients and families—especially older adults and those living with Alzheimer’s. Her heart has always been with the patient, working alongside case managers, social workers, and discharge planners to ease the process, reduce stress, and ensure no one feels alone in their journey. By removing barriers, she helps patients return home or move to the right level of care more smoothly, avoiding unnecessary hospital stays and readmissions. What makes it even more special is that these services come at no cost to patients or their loved ones.
